| Upon review of existing costs in preparing and finalizing various HKSTLA services, the HKSTLA executive committee has resolved that all existing costs will remain unchanged with effective from 1 January 2010, which is listed as per following: 2010 New Rate (Corporate Member|Chief Maritime Transport Operator (“CMTO”)|Fellow Member (“FSTLA”)|Member (“MSTLA”)|Associate Member (“ASTLA”)|Graduate Member (“GSTLA”)|Studying Member) New Member Entrance Fee to HKSTLA ($2,000 (one-off)|$500|$1,500*|$1,000*|$500*|$500*|$300*) HKSTLA Subscription Fee per Annum ($3,000|$500|$1,500|$1,000|$500|$500|$300) Member Company Name Change Fee ($1,000|Nil|Nil|Nil|Nil|Nil|Nil) Member Membership Change Fee ($1,000|Nil|Nil|Nil|Nil|Nil|Nil) Membership Certificate - Reissue ($500|Nil|Nil|Nil|Nil|Nil|Nil) CPD Cert Fee (Nil|$100|Nil|Nil|Nil|Nil|Nil) Remark: * Fee is waived if application is received before 15 October 2010. The special entrance & annual membership fee will be good through one year. You can join this community and take advantage of the many services we offer. HKSTLA members receive substantial discounts on: networking events, seminars, workshops, luncheons, visit tours and various training courses. By joining of Fellow Member ("FSTLA") before 15 October 2010, you will also receive a free admission ticket (Original price is HK$500 per head) to participate our annual dinner held in March each year. The basic requirements to apply for corporate membership in HKSTLA are: 1. The applicant * must be a Hong Kong based air/sea freight forwarding/LSPs company with limited liability and with over one year of operation. 2. Documents to support applicant’s financial background and the commencing date of business transactions (Annual Return including Schedule; Certificate of Incorporation; Certificate of Business Registration to be provided). 3. Please briefly to introduce your company (applicant) background such as set-up date, all director name & title no. of employee in H.K. and business nature using your company letterhead issue. 4. Please fill in the application form and send it (original) back to us. 5.
